Identity

Cagrilintide is a synthetic analogue of human amylin, also called islet amyloid polypeptide, engineered against the aggregation and instability of the native hormone and carrying a C20 diacid side chain.

Molecular target

The amylin receptor family, formed by the calcitonin receptor in complex with receptor activity-modifying proteins RAMP1, RAMP2 or RAMP3, giving AMY1R through AMY3R; the analogue also retains activity at the uncomplexed calcitonin receptor.

Signalling pathway

These are Gs-coupled receptors signalling through cyclic AMP. The published work localises the effect of interest to the area postrema and hypothalamus, where amylin signalling contributes to meal-termination circuits. The diacid side chain binds serum albumin, extending half-life to roughly a week in reported pharmacokinetics, which is why the analogue is used where native amylin is impractical.

Rodent and non-human primate feeding models, and an extensive clinical development literature for the compound alone and in fixed combination with a GLP-1 receptor agonist. Frequently paired with GLP-1 agonists in study designs because the two pathways converge on overlapping hindbrain circuits.
