The three receptors

The receptors concerned — GLP-1, GIP and glucagon — are all class B G-protein coupled receptors, and all three couple to Gs and raise cyclic AMP. Their tissue distribution is what separates them.

GLP-1 receptors appear on pancreatic beta cells and in the central nervous system, associated with glucose-dependent insulin secretion and appetite signalling. GIP receptors are found on beta cells and adipose tissue. Glucagon receptors are predominantly hepatic, associated with glucose output and energy expenditure.

Why combine agonism at three receptors

The glucagon receptor is the counterintuitive one. Glucagon raises blood glucose — apparently opposing the other two. The rationale in this class is that glucagon receptor agonism also increases energy expenditure and affects hepatic lipid handling, and that the glucose-raising effect can be offset by simultaneous GLP-1 and GIP activity.

So the pharmacology depends on the balance of potencies across the three receptors, not on any one of them. That balance is the defining property of any molecule in this class, and it is why two triple agonists are not interchangeable.
